Salary: £45,000 per annum, increasing to £50,000 following successful completion of probation, dependent on experience

Harland Care Services supports young people and adults with complex health needs to live as independently as possible in their own homes and communities.

We are looking for an experienced Regional Community Nurse – Complex Care to join our growing clinical team. This is a varied role combining clinical leadership, community nursing, staff training and clinical oversight across a regional caseload.

The Role

You will:

• Undertake holistic assessments and develop and review individualised care plans and risk assessments.

• Provide clinical oversight across complex care packages, ensuring safe, effective and person-centred care.

• Act as a clinical escalation point for clients, families and care teams.

• Provide clinical guidance, supervision and mentorship to Personal Assistants and nurses.

• Deliver and assess client-specific clinical training and competencies.

• Recognise deterioration and ensure appropriate escalation of clinical concerns.

• Support incident investigations, safeguarding, audits and quality improvement.

• Work collaboratively with families, commissioners and the wider multidisciplinary team.

• Support the mobilisation of new care packages and contribute to the ongoing development of our complex care services.

• Ensure care meets CQC, organisational and commissioning requirements.

This is much more than a traditional community nursing role. You will have the opportunity to influence clinical practice, develop staff and help shape high-quality complex care across the region.

What We’re Looking For

Essential:

• Current NMC Registered Nurse – Adult, Children's or Learning Disability.

• Minimum three years’ post-qualification experience.

• Significant acute hospital experience.

• Community care experience is essential.

• Experience with ventilation, tracheostomy care, PEG feeding, seizure management or other complex clinical interventions.

• Strong experience in complex care, clinical assessment, care planning and risk management.

• Experience delivering clinical training and competency assessment.

• Strong clinical decision-making, escalation and communication skills.

• Understanding of clinical governance, safeguarding and quality improvement.

• Ability to work autonomously and manage a regional caseload.

• Full UK driving licence and access to a vehicle.

Desirable:

• ITU / Critical Care / HDU experience.

• Teaching, mentorship or assessor qualification.

• Experience working with commissioners and multidisciplinary teams.
